What is the GUIDE Model?
The Guiding an Improved Dementia Experience (GUIDE) Model is a nationwide, voluntary initiative launched by CMS. It aims to enhance dementia care, reduce caregiver burden, and enable patients to remain in their homes longer through coordinated care and comprehensive support services.
What the GUIDE Model Offers:
- Respite Care Services – Temporary relief for caregivers to prevent burnout.
- Dementia Management Training – Education for caregivers on best practices.
- Support Groups & Counseling – Emotional and mental health support for caregivers and patients.
- Support Line – Ensuring round-the-clock assistance.
- Care Navigation & Coordination – Dedicated Care Navigators to facilitate comprehensive care management.
- Comprehensive Care Plans – Tailored for each individual, including assessments for cognitive, functional, and social needs.
How the GUIDE Model Works:
- Existing Patients: Ask your Neurologist about the GUIDE Model and your eligibility.
- New Patients: Call us at (760) 631-3000 to schedule an appointment.
- Consult: A Neurologist will review your eligibility for the GUIDE Model and refer you to our Memory Care Team for a comprehensive assessment.
- Comprehensive Assessment: Our Memory Care Specialists (MCS) will conduct a thorough memory assessment and create a personalized care plan.
- Personalized Care Plan: Our Memory Care Coordinator team will determine your eligibility and coordinate a tailored care strategy.
- Comprehensive Caregiver Support: Access training, respite care, and emotional support resources.
- Ongoing Monitoring: Our Memory Care Coordinator team will conduct regular check-ins to adjust care plans as needed.
- Care Coordination with Partner Organizations: We collaborate with hospitals, home health agencies, and community services to enhance patient care.
Who Qualifies for the GUIDE Model?
Important Notes:
- The GUIDE Model is a covered Medicare benefit, but your usual deductible and coinsurance will still apply.
- Your insurance benefits will remain unchanged.
- You maintain the right to visit any healthcare provider of your choice.
